#9 1996 Leaf Signature Extended Autographs Century Marks

As I recall when I got my autograph card there was a Leaf card that says you won the autograph card. Then you had to mail the card by XXX date or you would not get the autograph card. Then they mail the card to you. I have not seen the card since I mailed mine in. Do you have this card? Does anyone else remember the card or am I loosing it? or maybe another alternative is all of the above
