The one with only Juan on it is from 1998 and it commemorates his 200th homerun.
I have one still in the package, I can provide a scan if you'd like.

Dan has it on his list as 1996, does it say 1998 on the package? Thanks for the scan.

It didn't fit my scanner very well, so I included scans and a pic.

It says on the packaging "1998 Edition"
And on the Premier Replay itself it says "Gonzalez smacks 200th Homerun"

Your 1998 Concepts Premier is almost the same as the 1996-7 version. The only thing I see different is the Major League baseball stickers on the front and the fact that nothing is mentioned about it having a 5000 print run. I was told the one that I have was never put out in retail sales. They must of got em out in 1998 with little change.

Mine is in a clear plastic package with literally nothing on it. Mine is also numbered to 5,000.
